CARE IN YOUR OWN HOME
Sometimes care outside of your home may not be practical. This may be for a variety of reasons. In this situation, we offer skilled and unskilled personnel for private duties in the homes of those who call upon us. Whether its nursing for the chronically ill and/or vented patient or personal care, supervision, medication reminders and respite services for those with dementia or limited mobility, Meadows’ In-Home Division is equipped to serve. As with all services the process starts with an assessment. From there we develop a Plan of Care and commence service based upon individual need.
